SPECIAL DIETARY NEEDS

 U.S. Department of Agriculture (USDA) nondiscrimination regulation (7 CFR 15 b), as well as regulations governing the National School Lunch and Breakfast Programs, make it clear that substitutions to the regular meal must be made for children who are unable to eat school meals because of their disabilities, when that need is certified by a licensed physician. Copies of this form are also available at each school campus.

To remove a special dietary request from your student's account, a note signed by the physician stating that the student no longer has the food allergy or intolerance must be submitted to the FISD Child Nutrition Department. We may not change or revise a prescription or medical order.

Completing the Special Dietary Needs form does not automatically guarantee that an allergen-free meal will be available daily without request. It is the responsibility of the parent or guardian to review the menus and assist their child in selecting safe and appropriate menu options. Our online menus can be filtered by allergens. If your child is unable to make menu selections due to their diet restrictions, please contact Crystal Thill at crystal.thill@fairfieldisd.org for further assistance.

Special Dietary Needs Form

Physical impairments, including food allergies, that result in a severe, life-threatening (anaphylactic) reaction, and/or substantially limit one or more major life activities are considered a disability and an appropriate prescribed food substitution will be made with a licensed physician's assessment.


All other food allergies and intolerances not included in the scope of this definition are not identified as a disability under either section 605 of the Rehabilitation Act or Part B of IDEA.  The Child Nutrition Department, at their discretion, may make food substitutions for individual children who do not have a disability, but who are medically certified as having a special medical or dietary need.  The Special Dietary Needs form must be submitted and decisions will be made on a case-by-case basis.

Fluid Milk Substitution Request

Parents may request a fluid milk substitute be offered in place of regular milk when there is a medical or other dietary need.  This request does not have to be signed by a medical authority.  A milk substitute will not be offered or served unless we have this form on file for your student(s).  The type of substitute is at the district's discretion.
